Superb "Creepshow" 33 rpm vinyl record presented in an illustrated cardboard sleeve with very colorful graphics, typical of horror and comic book aesthetics. The visible side of the jacket features several stylized characters and creatures, including a large greenish-blue skull, a woman with a fixed gaze, and various unsettling faces, all against a graveyard backdrop and a nocturnal atmosphere. The title "Creepshow" appears in dripping green letters, immediately recognizable to horror film enthusiasts.

In the upper right corner, two edition stickers are present. The first, handwritten, indicates "2nd Press Purple Edition". The second, printed, mentions "Waxwork Records" with a green skull logo.
